The recent discussion on PhC in the hospital setting, must be a
real American one. In most European hospitals, as far as I know,
the emphasis is still very much on drug-management, and not on
patient-management. So, there is a totally different scope.
However, if we regard the activities of the European Society for
Clinical Pharmacy (ESCP), there is some hope. They slowly start
to get away from the drug-focus into patient-focus. And not merely
on the clinical pharmacy side of things (drug information, blood
levels, drug related errors), but also on the patient level (thinking
about patient convenience, individual outcomes of therapy,
communication etc..).
Although I think this is an excellend development, there is still a lot
to be done in hospital practice. Parmaceutical care, for the time
being, seems to play a much larger role in Europe in community
pharmacy than in hospital pharmacy
